Insurance Information
Innovis Health is pleased to be able to participate with many major insurance companies. We are continually reviewing plans and companies to be able to provide the best quality care for all patients in all locations.
Insurance:
You may have health insurance that covers all or some portion of the amounts owed by you to the clinic. The clinic has agreements with many insurers and governmental agencies that authorize the clinic to submit claims on the behalf of insured patients for the balances owed to the clinic. You are solely responsible for providing the clinic correct insurance information and must present your insurance cards to the receptionist each time you register for an appointment. Any co-payments required by your insurance company must be paid at the time of service.
Workers Compensation: If your claim is denied, you will be responsible for payment in full.
If your insurance company has not processed your claim after 60 days, the clinic suggests you contact your insurance company. Should your insurance company require another claim or additional information, the clinic will be pleased to furnish your insurance company the needed information. You remain responsible for the prompt payment of the entire amount owed to the clinic including all amounts that may not be covered by your insurance even if the clinic files a claim. If your insurer fails to make payment, or denies coverage, all or in part, you are responsible for filing your claim. The clinic does not negotiate settlements or undertake collection efforts from insurers after filing the initial claim.

Preauthorization:
Preauthorization
is required by most insurance companies before services are rendered. Contact your insurance to verify if authorization is required. Failure to obtain either preauthorization or a second opinion may result in your insurance company refusing to pay your claim. You will be held responsible for the billing.
Referral:
If you are seeing a doctor who is out-of-network, you are responsible for obtaining the approved referral
from your primary care clinic or primary care physician. Failure to do so may result in reduced benefits or no benefits from your insurance company, and you will be responsible for the billing.
