Hospital Registration
Due to differences in procedures and the differences in registering for these procedures, if you are scheduled for a procedure at Innovis Health, you will receive notification prior to admitting as to what time you should arrive at the facility, where you should register and what items you should have with you. To assist us in keeping our surgeries on schedule, and to allow time for you to get comfortable, please give yourself enough time when coming to register.
When we have a patient requiring immediate admission to the hospital due to critical situation, we offer bedside registration. This allows us to take care of our patients first and, when ready, gather the necessary information needed for proper registration.

Clinic Registration
Arriving for Appointments:
If you are an established patient
please arrive 15 minutes before your appointment unless you have been asked to arrive earlier. By helping us keep to the schedule, we can work together to avoid unnecessary delays. If you are accessing care related to a work injury, please inform the registration staff.
When you arrive for your appointment, you will be informed if your physician has been delayed or called away for an emergency. If you have more than a 20-minute wait before being called for your appointment, please check with the receptionist.

New Patients:
To help make your visit to the clinic easier, we’ve outlined some steps for getting acquainted.
First, if you have never been a patient at our clinic, please check in at the registration desk in the lobby 30 minutes before your scheduled appointment to pre-register. You may be contacted by registration staff prior to your appointment to be pre-registered by telephone. If you have pre-registered by phone, you need to arrive 15 minutes before your scheduled appointment time unless otherwise directed.
At the time of registration, we’ll need your medical insurance or billing card and the following information for your file:
- legal name
- current address
- birth date
- social security number
- name and address of person responsible for billing
- name and address for any special billing
- name, address and phone number of the person you want contacted in the event of an emergency
- you will also be asked to pay your co-pay if applicable for your health plan / visit.
North Dakota offers three low-cost or free healthcare coverage programs for eligible children. These programs include the Caring Program for Children, SCHIP/Healthy Steps, and Medicaid. Eligibility is based on family-size and income levels. If you would like information about any of these programs and/or an application mailed to you, please contact 1-877 KIDS NOW (1-877-543-7669). Scheduling an appointment:
Please schedule routine appointments in advance. Some physicians schedule routine exams months in advance for your convenience. Problems and emergencies will be handled promptly. When you call concerning an emergency, be sure to inform the scheduler of the emergency.
Same-day appointments may be worked into the schedule. Please be aware, however, that scheduled appointments do have priority, unless it is an emergency.
When you speak to the scheduler, they will ask you to describe your problem to ensure reserving an adequate amount of time for the appointment, and confirm that you are seeing the appropriate physician for your problem. Anything you tell them is confidential.
Canceling an appointment:
If you have a conflict or need to cancel an appointment, please cancel as far in advance as possible.
Occasionally, we find it necessary to cancel an appointment. In those cases, we will make every effort to reach you by phone to reschedule the appointment. However, if we cannot, we will send you a letter in the mail to keep you informed. If you are out of the local area, please use our toll-free telephone number to call us back to reschedule.
